If you would like a copy of your medical records for yourself, PRIOR to February 28, 2019:

1. Print, Sign, and Mail this form: PDF
2. Mail form to:

   • Coos Bay, OR 97420
3. Include a check for $25.00 made out to Oregon Coast Dermatology.

After we receive your signed form and check, we will photocopy and mail your records to you.

You can also stop by Oregon Coast Dermatology, and we can hand you this same form to fill out in our office. We will then process your payment, and mail your records to you.

AFTER February 28, 2019, your medical records will be managed by Desert River Solutions. You can go to their website and order your records for $25.00, or call them directly.

Of course, there are also other non-dermatologist doctors and practitioners who can take care of some types of medical and surgical skin problems, and you may wish to discuss with your primary care doctor or practitioner who would be best for you.

When you plan to see another doctor or practitioner, and need or want your records to go to them (or they need/want your records) you will need to sign their record release form. If it is prior to March 1, 2019, they should request the records directly from us. You should see a dermatologist when you:

– Have any particular concerns about your skin
– Find a lesion that you suspect could be skin cancer
– Have risk factors that increase the likelihood that you will develop skin cancer
Dermatologists are best qualified to evaluate and treat skin cancer because they receive extensive medical training in the diagnosis and treatment of skin conditions, including skin cancer. In advanced cases, a dermatologist may be part of a team of experts who treats the cancer.

Find a Suspicious Lesion:
People of all races and skin types get skin cancer. With early detection and proper treatment, skin cancer is highly curable. If left untreated, skin cancer can damage surrounding tissue and in some cases spread. Once it spreads, or gets very large, the prognosis can be poor.

To detect skin cancer in its early and most treatable stage, dermatologists recommend that everyone perform regular self- examination of their skin. Today, more than half of all diagnosed melanomas may be first identified by the patient. A self-examination is easy to do. Using a full-length mirror and a handheld mirror, check your entire body for moles and other lesions that:

Have grown
Bleed and crust over, but do not heal
Have changed, including the appearance of a new mole
Look unusual
If you discover a mole or other lesion with any of the above characteristics, don’t delay in making an appointment to see a dermatologist. You also should see a dermatologist if you notice a brown to black-colored streak underneath or bordering a fingernail or toenail, a pigmented mass in your mouth, or a dark spot on an eye. Remember, early detection could save your life.

Have Risk Factors? See a Dermatologist regularly.
If you have risk factors that increase the likelihood of developing skin cancer, you should perform self-examinations as well as see a dermatologist for regular checkups. These risk factors include:

1. Close blood relative has/had melanoma, several more distant relatives have a history of melanoma, or a family history of other skin cancers
2. Personal history of skin cancer
3. History of significant exposure to ultraviolet (UV) rays from the sun, tanning beds, or sun lamps — whether intermittent or year round, even if the exposure was years ago
4. Experienced severe, especially blistering, sunburns
5. Fair skin, especially when the person has blonde or red hair, and blue, green, or gray eyes
6. Sun sensitive, or tend to burn and freckle rather than tan
7. Large, asymmetrical, or unusual-looking mole(s)
8. 50-plus moles
9. History of x-ray treatments for acne
10. Taking immunosuppressive medications
